Preparing For Your Visit
New patients must call or email contact information to make an appointment. We will call you back to answer your questions and make your appointment. Patients who have seen Dr. Ferguson previously are considered established patients and may make on-line appointments.
For new patients and previous patients coming in for your first visit to my new practice, please complete the forms and mail, fax (#925.307.5742) or e-mail them back to office@sarahfergusonmd.com. It is important for Dr. Ferguson to review your file prior to your appointment. You should include copies of recent lab work and other relevant tests results or bring copies of the results with you to the visit.

Please bring all nutritional supplements you are taking. Bring a detailed list of all your prescription medications or bring the prescription bottles for all medications you are currently taking.
